SDEA bargaining unit members who did not sign up in spring 2008 for the voluntary 12-Pay Option will have an opportunity to elect 12-pay for the 2009-2010 contract year by submitting the following forms by Thursday, April 30, 2009.

- Debit Authorization

- Direct Deposit Signup Form
(only needed if not already enrolled in Direct Deposit)
- This option is available only to SDEA bargaining unit members who will be in active employment status and assigned to a traditional site on July 1, 2009. Temporary contracts scheduled to end prior to July 31, 2009 are excluded.
- Employees who are on a long term leave of absence through the end of the 2008-09 school year, have stated their intent to return to a teaching position for the ensuing school year and will not begin their assignment until September 2009 are not eligible for this option. Only employees in an active and paid assignment as of July 1 are eligible.
- Unit members assigned to year-round schools are automatically assigned to 12-Pay and do not need to enroll.
- Current pay schedule status is listed on paychecks at the top center. If “Pay Group” is listed as C13, the voluntary program is already in place. If C13 is not listed and you want to receive 12 pay checks beginning July 1, 2009, you will need to enroll.
- 12-Pay enrollees are required to enroll in direct deposit if they have not already done so.
- Incomplete applications will not be accepted. Make sure that both the New Enrollment Form and the Debit Authorization are signed and the bank account information is the same as your primary direct deposit account.
- Employees who moved from a year-round to a traditional site during the 2008-2009 contract year will need to apply if they are assigned to a traditional site and want to receive 12 paychecks in 2009-2010.
Termination of the Voluntary Option
If you elected the 12-Pay Option in spring 2008, it is not necessary to reapply if you wish to continue receiving 12 checks in 2009-2010. You will automatically be renewed. If you enrolled but no longer want to participate in the voluntary option, complete and return the following form by Thursday, April 30, 2009.
