Education Division Medically Homebound Program
Overview The Medically Homebound Program provides general education and special education services for students while they are confined to their home according to their physician or psychiatrist/licensed clinical psychologist recommendations. Special Education students will be served within the guidelines of their Individualized Education Program (IEP) The goal of the Medically Homebound Program is to provide education services and support in order to meet graduation requirements. Referral Process Nurses, physicians, psychologists, school staff, social services, or others may direct the parents to make a referral to the Medically Homebound Program. Parents request a referral packet directly from the Home Hospital Program (H/H) office. Packets are sent to parents, including a consent form to obtain medical information from the physician, psychiatrist, or licensed clinical psychologist. This form must be returned to the H/H office via fax or by U.S. mail. The H/H office will determine the student’s eligibility and notification of eligibility/ineligibility is given to the parent, referring party, and school the student attends. Special Education students will have their IEP reviewed prior to program entry. Other students may be referred for a special education assessment if the student status indicates this need. Eligibility The physical/health medical diagnosis from a physician verifies the severity of the diagnosis that prevents the student from attending school at the least restrictive environment. Emotional health diagnosis is verified by a psychiatrist or licensed clinical psychologist. Acute onset not chronic status is considered appropriate for the program’s educational services; Estimated confinement to home is greater than four weeks (20 school days) and usually less than six months. Programs and Services Homebound staff maintains close communication with student’s general education teachers to ensure the appropriate provision of services in order to meet the student’s educational needs. Currently, about 50 students and their families are served. Homebound students may be either special education or general education students; however, the latter are served when space is available. Special education students are provided with a maximum of 300 instructional minutes per week as physical condition and appropriateness permit, or as stated on the IEP. Other students may receive up to 180 minutes per week. Hospital Program staff liaison with parents, student, and school site to ensure a successful transition back to school once the student’s medical condition permits.
