I am the custodial parent/guardian and I have read the Program Policies. I understand that the selection process (except for PISC) is by an unbiased computer-generated randomized ranking procedure during the priority application period (Nov. 1 - Feb. 15) and that enrollment will be offered on a space-available basis by the school(s). I understand that if my student is enrolled, he/she must remain in the new school of enrollment until the completion of the school year he/she begins. The address used for eligibility will be verified with the district records at the current school and upon enrollment in the new school. I also understand that falsification of the student information may result in the student being disqualified for enrollment at the Magnet, VEEP, Open Enrollment or PISC school.
